Leveling subfloor before hardwood ??

I am installing 400 sq ft of 3/4″ prefinished hardwood, my problem is, one room’s floor is 34″ lower than the other, and I am joining the rooms together with the wood floor. How do I make the transition without it being noticable? The subfloor in the original part of the house is particle board, is it OK to install hardwood over particle board?

Which is higher ... the old or the new?
What is the floor structure of each part of the house? (joist sizes, sub-floor thickness, etc)

The addition I added was built 34" higher than the existing floor where it joins the house, I plan to put hardwood in part of the new addition and also in an adjioning room which is in part of the original house. House subfloor structure is 34" partical board over 1/2" plywood on 2 X 10 floor joist that are 16" apart. Addition subfloor is 7/8" tongue and groove OSB on floor trusses made with OSB that are 16" apart. If I just add 34" OSB over the partical board then I will have to deal with the transition somewhere else.
Hardwood over particle board doesn't work. If it's OSB, it does work.
Is the particle board section the higher part? If so, is it possible that there's a subfloor underneath the particle board? If so, it's possible that you could remove it and get the height the same.
