Look for flush mount simplex elec outlet
Can anyone direct me to a supplier of a simplex outlet that mounts flush into a surface (like a floor) and is 1/2 the face size of a standard duplex outlet? I want to put one on either side of the top of my fireplace mantle. It needs to be flush to the surface so it doesn’t stick out and it needs to be smaller that a typical duplex. I’ve seen flat simplex outlets but they are as big as a duplex.

Actually I think that it is the coverplate that you want to be small.
You might want to look at a clock hanger outlet. But I think that it will be the same size as it fits a normal box.
You can take any box and mount it so that the coverplate is flush with the surface.
Now you might want to make your own cover plate out of the same material as the mantle (If it is wood you want a thin metal plate under it). I think that you are stuck with something as big as the box.
Depending on the details you can get plug-mold and make a slot for it and paint it so that it blends in.
I think you'd have to get something made up special.
Outlets designed for panel mouting are available that you could mount to a square piece of metal or hard plastic. Then make up a small box, or use a regular sized box with a cover plate that transitions to a small square box opening. Be sure to plan how you'll screw the outlet plate to the box.

There is something available but i don't recall the manufacturer. Perhaps Arrow Hart would have it. I 've installed them before. The device is in two pieces and attaches to the end of a romex like a replacement cord cap. There is a little flange around the round face of the receptical to hold it darn close to flush . It has to be mortised in.
I just don't remember the brand. I didn't order them, the shp did. Hope that helps.
