When I open the tap, the faucet washer slips in its seat, doesn’t turn, and loosens the screw. I’ve tried new washers, new screws, threadlock on the screw threads, sandpaper under the washer- Can anybody help?

change the seat...

Wasn't sure of the terms. The washer acts like its hung up on the seat and doesn't turn

change or polish the seat.... (best bet)
change the style of the washer (might take care of the problem)
