I’m designing a home in the Sierras with some modern rustic touches. I love the look of exposed rafters in a vaulted / cathedral ceiling situation, but they don’t seem to make sense with the increased insulation requirements. It seems like you would need to use the rafter space, or is it realistic to get all of the insulation above the roof? I guess I could put up cedar boards below the filled rafters for the wood look. Just hoping to get a rambling discussion about vaulted ceilings, scissor trusses etc. I expect to have traditional attic spaces above most of the house, but it would be great to have some volume and wood on the ceiling in the great room and master bedroom.

Wood ceilings made with pine or cedar paneling give your cabin that traditional look. Being in their presence provides a warm and cozy feeling that is relaxing. Using all wood gives a consistent theme in the rooms throughout the cabin. -https://www.cozynestliving.com/
Exposed rafters with insulation on top of them.
Scissors trusses are usually the simplest and most economical solution to the insulation of a vaulted ceiling if you have enough pitch to make the ceiling look good.
