Can new tile be placed over existing ceramic? I have to re-tile some old backsplash areas to match new ones and sure would like to avoid ripping up the wall. Does the old surface have to be roughed up? What type of adhesive would be best?

You'll get a lot better result if you demo the old tile. Just a backsplash? That's not much work.

If your backsplash has any exposed edges you will want to get some kind of tile to finish off these areas, possibly regular bullnose but more likely a half or quarter round or rope, something to cover up the old tile edge.
It can be done but it's not as good of an installation. The old tile must be very sound.
