Who makes one, the kind that is compatible with a belt-drive rear-motor contractor-type tablesaw?
All I see for sale in the catalogs are either the ones that go onto cabinet saws (Unisaw, etc.) and direct-drive portables (like the Rousseau stands).
I know, I know. I should make one myself.

I made one for my contractor's saw.

Are you sure HTC makes one? I have only seen theirs that fits to a Unisaw or Unisaw clone.
Im pretty sure htc does....They have a list in their catalolue on which saws they will fit. They do show a cab saw in the picture
I was wrong they only make one for cabs saws sorry...but I know Rocklers does

Here's some pics of my outfeed table. Always intended to put a hinged section on to extend it further, but haven't gotten around to it yet. The angle iron under the side table ( right of the blade ) is thru-bolted to the table. I made a plywood hanger for the angle left of the blade. Its bolted thru the same holes the extension table is. I had to shim that angle for the table. The table itself is about 1/32" below the main table so materials don't get caught on the edge as they're being pushed thru.
