We are replacing our Cu water lines which are in the cement slab. I have a 2″ deep x 4″ wide x 20′ long trench produced in the slab to accommodate the PEX hot/cold lines. The plan is to put foam pipe insulation around the PEX lines and then fill in the trench with fresh cement before tiling. Is this a sound approach? Other suggestions? Thanks!

No, not a sound approach. Your trench should go through into the dirt otherwise the PEX and foam will compress and cause the concrete over them to break.
