I don't know where you live, buy I think your best bet is go to a local plumbing house and tell them what you need, if they don't have them a lot of times they will tell you where to get them. Luck.

look through to page 3 and 4 or so.
When you go to the Plumbing supply house (still the best bet), tell them you want retrofit, or two-piece, escutcheons. Unless you'd like to take the heating plumbing apart.
While you are there get some high-temperature pookie (that's a highly sophisticated "industry" term there--honest) to help stop the drafts around the pipes while you are at it. Oh, and a handfull of foam backer rod in several sizes (which sometimes means not needing pookie for air-sealing).
Local plumbing supply or HD has them
