When I read an article that mentions a house came in at $200 per square foot, do they mean house and land or just cost to build the house?

Kevin, usually it's just the cost to build the house, but the numbers are still funky--do you include the cost of the garage? Porches and decks? Basements and attics? If it's an addition, is an upgrade in electrical service needed? Fees, permits, engineering, design? You might be able to build a simple shell for $30 a sf, or a nice kitchen might run $500 a sf.
