We have an RSF onyx fireplace installed in an outside wall and want to add onto our house opening up the room and extending outward from the fireplace wall. All that we can think to do is to leave the fireplace where it is and open the room up around it. It will then be in the middle of the room. Does that sound feasible? Will it look weird from the back side? Any ideas?

I've seen quite a few fireplaces in the middle of rooms, its done all the time.
Ed suggested a good idea, could probably do a number of tasteful things.
