I have to attach a shower grab bar onto a tiled plaster wall. It is in an apartment and beneath the plaster is concrete. What is the best way to get proper anchorage in the concrete with plaster over top. Same goes for a concrete wall with tile backer board and tile on top?

Is there any void between the tile and the concrete (or anything soft that could be compressed)? If not, anchor directly into the concrete. You may need to drill a clearance hole through the outer strata to allow the anchor to be set, depending on the type of anchor.
