I bought a box of 1-1/4″ simpson RH screws when I bought their joist hangers, but I’m confused about when to use them. Do you use them only when attaching the hanger to a single 2 by X? What do you do when attaching one to a doubled up 2 by X?

Neither screw nor hanger cares how thick the member is, it attaches the same.

Simpson cares, and it does matter. They have fasteners that are recommended for all their hangers and they are not all 1-1/2" long.
IIRC Simpson recommends these screws for use with only some of their connectors, not all. Don't remember if hangers were included. Also, these screws can't be used with the latest PT wood.
