I need to replace a kitchen sink. Are their standard sizes or am I faced with replacing the counter top also? Existing sink is cast iron/enamel.

I am afraid there is no standard sizes in cast iron sinks. There are quite a few sizes you can choose from. If you don't want to change the counter put one in of the same size. What material is the counter? In good condition? You are talking about rim mount, right?
There are standard sizes, but that doesn't mean your sink is compliant with them.
If you post what size your sink is, I'm sure someone will let you know how much trouble you'll have finding a replacement.
