A family friend just bought a cool little house that was built by a disciple of Frank Lloyd Wright, and it has Concrete block wall thoughout the interior, it looks like the block has been painted over a few times, and the paint seems to be adhereing to the walls quite well. My friend would like to skim coat the walls to make them less institutional, and I am wondering how to do it.
Is there a product that will go right on?
Do I need to secrure metal lathe up for the next product to secure to?
Should we try to remove the paint
or, should it just be left alone?

There are block filler epoxies available as what one might find in schools or public buildings that want to disguise the block. Don't know how they work tho' as I have never used them.

If the walls are slick / glossy surfaced, you need to sand enough to scarcify them so new surface will have 'tooth' to adhere. Then use an acrilic bonding agent such as made by Thorocrete or Arcril 60 T prime it. Then you can use plaster or durabond

I'm thinking the sand stucco might be too rough for interior walls, but that would work too if he used the bonding primer.

my friends are hoping for a smooth surface, but I am wondering about how flat the current walls are... I noticed some evidence of settling/walls cracking... Instead of sanding, do you think I could abrade it with a grinder and wire wheel? or flap sander on grinder? or just a plain random orbit sander?

don't go there. nasty dust! A good trowel or float man can skim them flat just fine. if they continue settling or moving, cracks will telegraph trough again.
