Does it include any masonry repairs (I’ve sometimes done when installing a door in a brick opening)? Does it include adding extension jams? Does it include hauling away the old door? Just wondering what the “standard” install includes and what’s extra…

One thing I've learned in last 30+ years in construction; there is no standard. At least not contractually. We all have different expectations.
What does your contract or bid proposal define as the scope? Is the work for a homeowner/client or a contractor.
I personally would have included in the bid: 1) remove the existing. 2) install the new. 3) haul off the old. 4) install extensions if necessary. 5) case the interior. 6) install lockset. 7) and make any repairs if I created damage.
Sorry to get personal, but "install" is a verb not a noun. Just a pet peeve of mine.

While I do agree with TKH (or was that THK?), there are no "standard" installs...anything falling outside the realm of carpentry I would consider "extra"....i.e. stucco/plaster repair, painting, doorbell rewiring...try and include everything you intend to attention in your estimate....leave line item extras for those tasks undetermined or outside your area of expertise.
