URL needed toilet height specification
Some knucklehead just mounted ALL the wall-mount toilets in our restrooms too high (not just the ADA stall). They measure between 18-19″ from the floor to the top of the ceramic, and add one more inch for the seat to that. I tried to point this out to the project manager, who said he would go back to the installers and find out from them what happened. Anyone got a URL I can go to and get what might be construed as a “standard” for toilet heights (aside from pointing out that all normal toilets are around 14″ hight?).

All these require Adobe Acrobat.
This is just one specific Am. Std. model's instruction sheet. It calls out 15" or 17" (ADA) rim height AFF.

This is one specific Toto. It calls out 15" AFF as 'suggested dimension'. Says it will meet ADA & ANSI A117.1 "...where installed so top of rim is 17" from the finished floor."

You can likely find similar documents for any commercial toilet. So, what're you installing? Also, what's code official say?
If everything seems to be going well, you've obviously overlooked something.
You need to search for ADA specs as well.
Best to you... sounds like a horrible mess to me!
Since they are so far from the standard you may very well be able to make the case that this finished product in not usable. If they won't make it right talk to a good construction attorney. But give them a chance to fix it first.
But don't accept it either.
PS. I am a Master Plumber so I am not just spouting off uninformed opinions here. But don't go say a plumber on the internet said so and so!
