what can i do with a big ol’ (24″x18″) metal return air register right in the middle of the floor in my 1940-ish house? its ugly and it hurts my bare feet! does it need to be this huge?

put it in the wall and duct it in....
what's hooked to??? basement or crawl space unit...
can you use a joist bay for the return duct to the perimeter of the room or wall???
wear shoes or slippers more often...

Is it still in use? Connected to the furnace return?
If so, you can't do much until you provide an alternative return, and that does NOT include just leaving/creating a big opening in the return near or at the furnace.

Are you sure it is a return ? Most of the ones I have seen in the floor are actually the hot air "outlet" for the system.

Floor furnaces had a big heat vent.But the old gravity (octopus) furances had a bir floor return right over the furnace..

If it's really a return, you can replace it with individual returns in each room. Then floor that one over.
Lots of new ductwork, but the system will look and work better.
