One place you might look is at your state Atty Generals office or Dept of Commerce. For example, the State of Maine posts a sample on the web for us that contain the necessary language to protect the consumer.
Normally, the contractor proposes in his own contract lanquage that he ahs worked over with his atty.

The builder should provide you with his contract, specifying all what he is going to do, dates, materials/techniques used, names of subs, etc.
You better run it by your attorney to change the few things that may be too much in the builder's favor, past customary practices for your area, if any.
This is totally unnecessary in the eyes of some and justly so between honest parties that are of one mind but since that is not the relationship most have with a builder and misunderstandings may happen, it is better to have all in writing up front.
A good contract will help if someone get's in trouble with paying or an accident happens.
That attorney's fee is money well spent.
