Installing a Lennox Direct Vent Fireplace Model MPD-33. Installation instruction say “route a 3 wire 120Vac 60HZ 1ph power supply to the appliance junction box”. 2 questions:
1) What size/type of wire is that? 12/3? 10/3.
2) What size breaker do I need?

Your saying there is no "plate" that says how many amps the thing draws? Sounds strange...
There should be a nameplate somewhere on the unit that says how many amps it requires. It should be buried somewhere in the owner's manual as well. If you can't find it, you should call the place you bought it or the manufacturer to find out.
I haven't installed any Lennox, but have put in a few Vermont Castings.
Unless the Lennox has some aspect that is extraordinary, the 120v. is to operate the blower and maybe some simple electronics for a remote control and the pilot.
The amperage requirements are relatively low.
While we run 12 gauge in most of our projects, I would bet that 14 would suffice with a 15 amp breaker.
But, as was mentioned, contact the dealer or scrutinize the installation instructions; that should be spelled out.
What wormie said. Unless the nameplate calles for something else, which is unlikely.
It'd be a good idea to involve your electrician at this stage, NOT after the 'rock is up and painted.

I ditto Wormedrive.
I had this exact discussion with my local inspector from the Ontario Electrical Inspection Authority and he was not too concerned about the fireplace even needing a dedicated circuit.
Have a good day.

How many amps (or watts) does it draw? Should be in the specs or on the nameplate.
It doesn't hurt to have oversized wire (within reason), but it's best to go with the "right" size breaker, to minimize damage in case of a short.
